Electronic Consent

CAPTIV8 Connect offers full electronic consent with version control.  Consent may be performed in 2 ways: 

  1. Consent only: where an uploaded consent document is annotated electronically and saved.  
  2. Consent with Quiz: where a consent animation playlist is shared which contains a quiz to demonstrate patient understanding.  The results of the quiz are automatically attached to the consent document and then annotated electronically.

Uploading a Consent Form & Adding Merge Fields

Before electronic consent can be performed, a consent document (in pdf format) must be uploaded in the 'Uploads' area.  Once the document is uploaded, click on the document and when the pdf viewer loads the pdf, you may add merge fields from the patient record.



Consent only

A consent document may be completed face-to-face with the patient.  The video below, describes how to complete a consent document only.



Consent with Quiz

One of the key pillars of consent is to demonstrate patient understanding.  CAPTIV8 Connect facilitates this by allowing you to share patient education animation playlists (which may further include your own uploaded videos) combined with MCQs interspersed between videos.  This aspect of consent is performed by the patient at home after sharing a consent platylist.  The video below shows how to create a quiz and consent playlist.  The video also demonstrates how to complete the consent with annotation in a face-to-face scenario wit the patient.
